Informed Consent


INFORMED CONSENT


DLJ Consulting operates under the assumption that all individuals who provide responses to items in the Diversity & Inclusion Professional Development Survey have given their informed consent to participate in the process. 


This is the fundamental concept that underlies all current and anticipated data protection protocols and legislation. In order for individuals to give their informed consent, they must understand the purpose of the process, the likely use of the data, and how the data are protected. These protocols are described below and are binding on all clients and related individuals. Failure to comply with any of these safeguards will constitute grounds for termination of any data transfer arrangements between DLJ Consulting and the person(s) or organization(s) concerned.


Purpose
The web-based Diversity & Inclusion Professional Development Survey was created and adopted to provide organizations with aggregated data related to the attitudes and behaviours of its employees, staff, students, and others  in relation to diversity and inclusion related practices.  The survey also requests demographic related information of respondents so that results can be interpreted more meaningfully, however provision of personal information by individuals is optional.  The aggregated nature of the reporting process also means that individual results are not identifiable.  The attitudinal, behavioural and demographic data collected are primarily used for, but are not limited to, the review and enhancement of the client organisation’s diversity and inclusion strategy.


Data Use and Storage

Data collected through the Diversity & Inclusion Professional Development  Survey is only provided to people within the client organization who are appropriately qualified to interpret such information.  DLJ Consulting will retain raw data for a minimum of three years on its service providers secure web platform. In addition, by participating in the survey, the client organization and participate agrees to DLJ Consulting using their aggregated (de-identified) data for research purposes.


Access to Data
The client organization and participate has access to all reports generated from their data by DLJ Consulting.  Dissemination of such reports by qualified personnel, who are responsible for complying with national and international protocols covering data use and storage, is at their own discretion.  Access to individual responses is not possible given the aggregated nature of the reporting process. DLJ Consulting cannot be held responsible for any litigation arising from the misuse of the reports (and data contained therein) emanating from the client organization’s actions.


Data Security
In order to safeguard individual results, the DLJ Consulting website contains only the survey items, not the assessment programs. It is impossible to process results through the website accessed by respondents to provide ratings. Results can only be processed by DLJ Consulting after downloading the raw data, decrypting the raw data, and scoring these data with appropriate programs.  While all respondents are provided with a unique link to access the survey, the raw data collected from respondents are encrypted.
